At the University of Chicago Hospitals Physical Therapy Department, we needed an organized hand-assessment form for recording the information necessary for effective treatment of hand injuries. We reviewed leading resources to identify the necessary data to obtain when evaluating hand function.1-4 The form we devised has seven categories for assessing a traumatic hand injury. Included are a brief history of the injury; measurements of active and passive range of motion of the wrist and all digital joints, including thumb opposition; and measurements of pulp to palm and thumb/index web spaces. A thorough section on hand sensibility contains hand diagrams on which the therapist may shade in areas of absent or dulled sensations. Space is provided for any additional comments the therapist may have.
