Correction and Revised Appendix

In February 2001, Physical Therapy (PTJ) published “A Comparison of a Modified Oswestry Low Back Pain Disability Questionnaire and the Quebec Back Pain Disability Scale,” by Fritz JM and Irrgang JJ. Appendix 1 of this article contained a modification of the original Oswestry questionnaire published by Fairbank JCT, Couper J, Davies JB, et al (The Oswestry Low Back Pain Disability Questionnaire. Physiotherapy. 1980;66:271-273). Prior to publication, PTJ had requested and received the permission of Physiotherapy/The Chartered Society of Physiotherapy, the publisher of the 1980 article by Fairbank et al, to modify the Oswestry Disability Index (ODI) contained in the 1980 article. It has come to our attention that Fairbank and his co-authors hold the rights to the ODI. Accordingly, PTJ has revised and reprinted Appendix 1 of Fritz and Irrgang to change its title from “Modified Oswestry Low Back Pain Disability Questionnaire” to “Modified Low Back Pain Disability Questionnaire.” We request that, going forward, authors and readers use this revised version of Appendix 1.
