We would like to thank Ms Binkley for her commentary. As she correctly points out, there are certainly limitations to the methods we used in this study, and these limitations apply to a greater or lesser extent whenever a clinical database is used to examine predictors of outcome. On the other hand, as noted by Ms Binkley, randomized clinical trials in the study of low back pain have their own set of difficulties and can be prohibitively expensive. Ms Binkley identifies some of the variables that may have affected our results for which we have no data. Insofar as is possible, those designing clinical databases should consider including such information. In addition, our study highlights the need for complete data collection and follow-up, and the problems encountered in attaining this goal....
