The majority of health care occurs out of the clinician's direct observation. Thus, a great deal of health care is self-care. For a treatment to be of clinical use, at least two conditions must be met: (1) The treatment must include a component(s) that has been demonstrated to be effective, and (2) the treatment must be administered appropriately to a patient or used in a prescribed manner by the patient. The history of health care has focused primarily on the first condition, establishing the validity of specific treatments. Only limited acknowledgment and systematic attention has been given to the latter condition, namely, compliance with the prescribed regimen....
